HTM Consultants evolved over the years when I was struck with a heavy heart when my late father, a former US veteran who was exposed to Agent Orange, fell ill and eventually proceeded in death at the age of 61 from cancer. His efforts of seeking help was disappointing and without success. I keep his memory alive by devoting my time to helping others in our community and nationwide through advocacy services.

When human beings experience medical deficiencies, trauma or severe life stressors, it is not uncommon for their lives to unravel. My great passion is to bring healing to people who have been through a medical deficit or stressful experience. I have assisted my clients/patients, who include children, adults and families, to find healthy ways to achieve a health management plan that addresses their interrelated medical, psychological, social, behavioral, educational, and financial needs to achieve optimal health, wellness, or end-of-life outcomes.

Certifications to include Motivational Interviewing, Educational Resource Management, Engagement in Community, Psycho Therapy, Smoking Cessation and several Chronic Diseases Management Awareness to name a few, to meet the needs of each client by providing an individualized plan of care.

My background consists of outpatient psychiatry, community nursing, corrections, home care, outpatient orthopedic, multiple insurance plans and long term care facilities. My educational background includes Licensed Practical Nurse, Medical Assistant, Medical Billing and Claims Specialist and Diabetic Shoe Fitter. My career background includes Psychiatric Nurse, Correctional Nurse, Case Manager, Independent Nurse Provider, Community Health Worker, Independent Nurse Consultant, Hedis Nurse Auditor, Risk Adjustment Medical Coder and Orthopedic Medical Assistant. I have extensive knowledge and experience as a small business owner. I am a previous owner of a Medical Billing Company for 15 years, a Home Health Care Agency for 4 years as well as HTM Consultants. I have been involved in case management for the last 12 years.

We know that no single approach is the right one for every individual. We have worked on many initiatives focused on improving clinical outcomes and improving patient satisfaction.

Being students of life has afforded our team the ability to always lend a helping hand to others, confident that something positive will be gained from these experiences. HTM Consultants serves our Clients and their Health Plans by providing their members a deeper level of understanding of their needs through affordable access to preventive care. Using a patient-centered approach, our providers seek to understand the current state of the members’ health and support them in setting mutual, realistic goals. As a progressive leader in Wellness Assessments, we are seeking credentialed, reliable, and caring CHHAs or CNAs providers that are interested in working in the Wellness Assessment field. We are offering IMMEDIATE Per Diem, 1099 positions across the United States to contact members of Health Plans who are in need of a short Health Risk Assessment. Delivering quality and convenient healthcare requires a passion for understanding members’ needs and subsequently assisting them in taking preventative measures to mitigate current or future health risks. POSITION SUMMARY
The assessor is expected to demonstrate proficient skills that support our member “Wellness Assessments”. The position may include work in a clerical, administrative, or clinical assessor capacity, and may be asked to perform more specialized tasks. Assessor will contact members of Health Plans who are in need of a short Health Risk Assessment. Assessor will contact the member by phone to complete a Health Risk Assessment or face to face if failed attempts to reach by phone. Once contact has been made, a Health Risk Assessment will be conducted.

ESSENTIAL JOB FUNCTIONS
• Welcome members by greeting them, in person or on the telephone • Verify member information by interviewing member; reviewing and recording medical history; confirming purpose of visit • Provide appropriate member education on medication, diet information and instructions; ensures questions are addressed or referred within scope of certification • Complete records by recording member assessment information and emailing to a secure email encryption program • Maintain safe, secure, and healthy work environment by following, and enforcing standards and procedures; complying with legal regulations • Maintain member confidence and protected health information (PHI) by keeping member care information confidential • Adhere to professional standards, and HTM Consultants policies and procedures, federal, state, and local requirements • Completes assessments • Other duties as required

POSITION REQUIREMENTS
• High school diploma or equivalent
• Current certification/nurse license
• Maintains current CPR certification and Annual TB screening
*Laptop computer with Windows 7 or above or OSX 10.9 or above
*Federal criminal background check within last 12 months
MUST BE DEPENDABLE/RELIABLE AND ABLE TO WORK INDEPENDENTLY

Required Skills and Abilities
• One (1) to two (2) years’ experience in the healthcare field and medical specialty • Knowledge of medical terminology • Highly effective interpersonal and communication skills; ability to communicate to all levels throughout the organization using written, verbal and listening skills • Demonstrated ability to accurately document a member’s medical history and to report technical information to other doctors and patients; establish and maintain records, reports and statistical data • Outstanding teamwork and multitasking skills. Ability to thrive and prioritize in a dynamic, rapidly changing environment • Dedicated to meeting the expectations and requirements of both external and internal customers, acts with customers in mind by exhibiting professionalism, empathy and courteousness • Passion for working with members/patients, changing lives with a comforting bedside manner • Consistently demonstrate a high level of professionalism, demeanor and confidentiality by using initiative and judgment • Demonstrated ability to meet deadlines, and use time effectively and efficiently. Travel Requirements
• Must meet company driving standards
• Must have valid state driver license
• Must be able to independently travel to assigned locations within a minimum 30 mile radius of your home address

Compensation
* 1099 Position - Independent Contractor
* Paid once per month
* Paid per assessment:
$8 if all attempts completed and not successful at connecting with member
$10 if all attempts completed and member located but assessment not completed
$15 if all attempts completed, member located and assessment completed

NOTE: There is NO mileage reimbursement at this time; Maximum of 50 assessments given at a time and all attempts to be completed within 10 days once assigned.
